Accessibility request forms

Accessibility Services uses a platform called Accommodate to securely store documents and manage student requests.

Many of our previous PDFs and Word documents have been migrated into Accommodate to streamline requests and services. Please log in to Accommodate from your MyAU portal under Resources > Support Resources. You will need your AU email address to log in to Accommodate.

New to Accessibility Services?

  • Complete the first of 2 steps to register with our office with your self-assessment of need.
  • Submitting this VOA requires attaching medical verification of the disability's impact. You may print the Medical Verification Form below.
• Medical Verification
  • You may provide this form to a medical professional who is verifying the impact of your disability on online learning.
  • Consult our summary of disability documentation page to understand accepted medical verification.
  • Once uploaded with your VOA or to the Documents section of your Accommodate dashboard, this completes your application for registering with Accessibility Services.
